RE: Participation in HBA of Greater Dallas Health Benefit Trust
For the
last two years, HBA Members and Staff have been working to create a Health
Benefit Trust in order to bring a unique and highly competitive health
benefit program to the principals and employees of our member companies. We are
at last approaching the time when we will be able to offer a top quality program
which will provide our members with many options for coverage and the
opportunity to add other elements to their package of benefits.
We have selected the service providers for
the program and are in the final negotiations to lock them into contracts. We
will be using the United Healthcare services for accessing the physicians,
hospitals and clinics, the pharmacy program will be administered by Medco, a
large national service company, the Dental and Vision programs will be serviced
by Guardian. You can check with your current doctors and dentist to see if they
are a part of these groups. You will find the health rates posted in the tab
section on the upper right hand side and the dental and vision rates are posted
in the description tabs for each of those programs.
Now, we
need your help.
If you would like to participate in this program we need to accumulate the data
contained on the form that is available on the HBA Mewa website.

When you get to the website
select the “Survey
Form” tab found at the top of this page. Print this form
out and complete it for each of your employees. If you have quite a few you
can develop an excel spreadsheet with the appropriate headings for each
column or use an AscII format file.
Once you have completed this information form or an excel spreadsheet of
your own design, please email, FAX, or snail mail it to:

Next, you should download the sample Insurance Carrier Contact form letter
found under the tab at the top of the page entitled “Insurance
Carrier Contact” on the website and follow the instruction as stated.
You can forward the returned contact form to the trust when it is received.
Time is of the essence, so your timely return of the requested information
will make the program available more quickly. You can read more about the plan
design on the website as well as the prospective premium levels.
